Policies and Informed Consent
Informed Consent means that you understand what I'm offering, what therapy might do for you and what the few exceptions to confidentiality are. My Informed Consent form also includes my office policies, like my cancellation policy.

The Consent to Release Information form is used if you would like me to talk to another person about any aspect of our work together. We would talk about it before I contacted anyone else.
